REQUEST FOR APPLICATIONS
Welcome to the online application portal for the Los Angeles County Department of Mental Health (DMH) Enriched Residential Care Plus (ERC+) Request For Applications (RFA). Complete information about this funding opportunity can be found on the Brilliant Corners website at https://brilliantcorners.org/ERC_RFA.
Application deadline via this online portal is no later than 11:59 p.m. Pacific Time on September 5, 2025.
Applications may not be hand delivered or mailed.
You do not need to complete the application in one session. You can save your work, exit and continue later. Once complete, you must click “Submit” in order to submit your application. If you are applying to operate multiple interim housing sites, you must submit a separate application for each site. Applicants are encouraged to draft responses based on the Application Questions Worksheet and to cut and paste text from that document as helpful.

- Enter your agency & site name to name your application
- If you don’t already have a Survey Monkey Account then you will need to register by entering your email address to create a login and password
- Check your email to verify your email address & click link
- Verification email will come from noreply@mail.smapply.net
- Start filling out application tasks
- There are ten (10) application tasks that you must complete before submitting your application
- Each task will include specific instructions for completing it
- Each task must be marked as “Complete” when done
- Once ALL tasks are completed, you must click “Submit” to submit your application
- You will be given an opportunity to Review your tasks before you submit it
- Applications that are not submitted will not be considered
- Applicants must submit a separate application for each proposed site, regardless of common owner or operator
